When a referral should be made
If you feel you are concerned for your child whether at home or at school, please feel free to make contact with the SWIS worker for your childs school to discuss. Alternatively you can contact our office on 07 307 14 72 and ask for a SWIS team member.
Who can make a referral
Whanau, the school or the child themselves are able to make a referral to the SWIS programme. Referrals can also be received from Community Organisations, Health Professionals or Child Youth and Family Service.
How to make a referral
To make a referral, please contact the Social Worker either via the schools listed or contact our office on 07 307 1472
How to access a referral form
Please feel free to contact our office on 07 307 1472 for a referral form which can be delivered by a SWIS worker to you.
The Social Worker MUST have the parent/guardians consent to work with the you and your child.
