Community Based Diabetes


Our Diabetes Service Provides:
-  Education within the community for small and large groups at workplaces, in homes or other places within the community.
- Support of people with diabetes to maintain lifestyle changes in their home.
- Help with access to other health services.
- Advocacy and support of Whanau/Family.
- Diabetes testing within the community.
- Assistance to liase with appropriate health agencies and services

- What should a healthy eating plan include
- Enjoyable daily physical activity
- Successful at being smokefree
- Monitoring blood sugar levels
-Testing at home with a blood glucose meter
- Foot and eye care
- Managing sick days and hospital visits
- Minimising long term damage from the complications of diabetes
